December 14, 2010 – Networking and Holiday Get together
Location: Malta Community Center, 1 Bayberry Drive, Malta, NY

**We will hold meetings on the 2nd Tuesday of every month 2:00-3:30 p.m. unless otherwise indicated


“Our mission is to promote community awareness of the programs and services available to Saratoga County residents who are aging and/or disabled, and their caregivers, so as to better meet the needs of these ever increasing populations.”


Introductions & Announcements

• Alzheimer’s Association Capital Region Memory Walk to be held on Saturday 9/25 at Saratoga Gaming and Raceway. 9 til 10:30 am registration, 10:30 walk kickoff, refreshments at noon. For more information, http://capitalregion.kinters.org.

• Evergreen Adult Day Services has moved to 357 Milton Avenue, Ballston Spa. Hours are 8:30 AM – 4:30 PM. On September 21 there will an open house from 4:30 – 6:30 PM. Offer free trial day for clients.

• Saratoga Vital Aging Network is partnering with Saratoga County Office for the Aging New York Connects for a conference to be held on October 20, “In My House, About Me, Without Me”. A Saratoga County Community Conversation on Diverse Pathways to Alternative Living Options for the Elderly. The conference will be held at The Holiday Inn, Saratoga Springs.

• CDTA - 40th Anniversary $0.40 a ride on September 16th

• Heartoberfest – September 18th.

• Saratoga County Board of Supervisors free concert at SPAC September 25th @3:00

• JCC Presentation on Health Care on October 19th

Speaker- Cindi Lisuzzo, RN, BSN, Director of Care Management, Saratoga Hospital
Saratoga Hospital Care Management
The care management team work with physicians, nurses, physical and occupational therapists, nutritionists and clergy. Care transitions – creating toolbox to decrease readmissions. Track and trend outcomes, share monthly data, create focus groups to analyze, collaborate with cross setting providers and personal follow up.


Speaker- Stephen Wright, RN, BS, Cardiology Program Director, Ellis Hospital
Ellis Medicine Transition of Care Program
Vision is the Chronic Care Model, 1 in 4 or 5 patients are readmitted to hospital for same reason.
Colena Model – follow up, personal health, medication and recognizing warning signs.
Creating partnerships – Home Care and Skilled nursing facilities.
Readmission task force – focus on reducing readmission and work on communication.

Health Care Reform
Affordable Care Act
2009 grants created Community Supports Navigator Program – pairs older adults with trained volunteers following discharge – follow up phone calls.
There are two pilot programs in Albany County and Tompkins County.
Albany County – Collaborative Model: necessary collaborations with Hospital, NY Connects and Professional Volunteer Organizations.
Tompkins County – Volunteer Only Model
Overall evaluation of the CSN Programs in both counties will be conducted by the Center for Excellence that will assist in the potential growth and expansion of the program.
