Cindi Lisuzzo from Saratoga Hospital and Sandi Cross from Saratoga County Office for the Aging will be presenting on the Community Based Care Transitions Program (CCTP) through CMS. Ten community hospitals and six community based organizations (CBO's), including Saratoga Hospital and Office for the Aging, have joined together to deliver care transitions specifically using the Coleman Care Transitions Intervention Model. The purpose of the model is to help patients help themselves by coaching them to recognize "red flags", equip them with better tools during and following the discharge process and to coach them with medication matching. The model is designed to reduce readmission rates by empowering patients to take charge of their healthcare.
