The next Aging & Disability Network meeting is scheduled for
Tuesday, April 9, 2013 from 2:00 - 3:30 p.m. at Saratoga United
Methodist Church, 175 5th Avenue, Saratoga Springs. Cindi Lisuzzo,
RN, BSN, Director of Care Management for Saratoga Hospital will be presenting
on the topic of Discharge Planning 101. Please extend the invitation to
