The next Aging & Disability Network meeting is
scheduled for Tuesday, August 12, 2014 2-3:30 p.m. at St. Edward the Confessor
Church, 569 Clifton Park Center Road, Clifton Park. Colleen Sheehan, Training and Technical
Assistance Facilitator for NYAPRS, will speak about Person Centered Planning
(PCP). PCP is directed by the individual
who owns the plan, with a focus on promoting recovery and community inclusion. Colleen will discuss O'Brien's 5 Valued Outcomes, core values and hallmarks of PCP.
Our next meeting will be held on Tuesday, September 9,
2014 2:00 - 3:30 p.m. at Malta Community Center, 1 Bayberry Drive, Malta.
Presenters will include a panel of providers from cancer related services.
