Aging and Disability Network Meeting Minutes 9/13/16

Present: Renee Birnbaum (Choice Connections), Tena Bunnell (Home Helpers), Whitney Jobmann (SAIL), JoAnn Zales (National Grid), Jennifer Baldwin (Saratoga Hospital), Mary Rickard (Saratoga County OFA), Emma Schmitt (Saratoga County OFA), Kim Swire (Retired), Sharon Emerson (St. Peter's CHOICES), Mark Quail (Albany Med), Kendal Pompey (Albany Med), Diana Martin (Wesley Evergreen), Denise DiNoto (Consumer Directed Choices), Cindy Harrington (Shelters of Saratoga), Reed Lehan (Saratoga County DSS), Maria Geizer (Home Instead).                 

Attendees introduced themselves. The next meeting of the Network will be held on October 11: Saratoga United Methodist Church, 175 Fifth Avenue, Saratoga Springs - Eve Bankert of IPRO will be presenting on the Community Based Sepsis Awareness Initiative.
November's meeting will be held on 11/9 at St. Edward's Church in Clifton Park. Mary Rickard will be presenting an overview of the Office for the Aging.

Today's Presenters: Mark Quail and Kendal Pompey, Program Managers from the DSRIP Program of Albany Medical Center Hospital. Presenting an overview of DSRIP (Delivery system Reform Incentive Payment Program).
DSRIP´s purpose is to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years. Up to $6.42 billion dollars are allocated to this program through New York state with payouts based upon achieving predefined results in system transformation, clinical management and population health. Care Integration for patients is achieved in part due to Value Based Payments - paying providers for value instead of fee for service. This also rewards performance positive outcomes and can control healthcare costs.
In our local counties, Albany Medical Center Hospital is part of the Performing Provider System, covering Albany, Columbia, Greene, Saratoga, and Warren Counties. In these counties there are more than 180 providers, including health clinics, behavioral health providers, hospitals and social services.
Goals are not only financial, but mainly to encourage patients to be more active in their treatment, and providers work to meet patients where they are, actively listen and treat underlying causes, not just symptoms, hopefully decreasing avoidable hospital use.
