Attendees introduced themselves and made announcements. The next meeting of the Aging and Disability Network will be held on 2/14 from 2pm-330pm at the Saratoga Springs United Methodist Church. The presenter will be Kym Hance, Program Manager with the Alzheimer's Association. The topic will be "Healthy Living for your Brain and Body; Tips from the Latest Research."
The March meeting will be held on 3/14 from 2p-330pm at St. Edward's Church in Clifton Park. Presenting will be Victoria Clingan from The Brain Injury Association on the topic "Brain Injury 101."
Today's Presenters: Jess Flynn, Community Liaison for The Eddy DayBreak Adult Day Services and Diana Martin of Wesley Evergreen Adult Day Services. Each gave an overview of their programs.
Wesley Evergreen is a social model day program for adults located in Ballston Spa. The program is designed to serve the needs of people who may have difficulty with tasks of everyday living or be partially dependent on others due to conditions such as dementia, depression or Parkinson's. They may live alone and need socialization, or are living with caregivers and need supervision during the day. This model allows families to have respite from caregiving and gives them peace of mind that their loved one is safe.
Evergreen is funded in part by the Saratoga County Office for the Aging and other community agencies. Transportation is available within a certain geographical area for an additional fee. The current fee, $45, includes lunch and a sliding scale fee structure is also available. Long Term Care may also fund program costs. As staff are CNAs, personal care can also be provided if needed.

Daily sittercise
Medication supervision and assistance
Caregivers' support group
Personal care
Recreational activities
Community outings
Health monitoring

Eddy DayBreak is an innovative adult day service that helps improve quality of life for seniors who still live independently in the community, or who are cared for by family members.
DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home caregiving so they can fulfill work or other obligations.
DayBreak services include:
- skilled nursing and preventive medical care
- assistance with bathing or showering
- social work services and care coordination
- rehabilitation services, including physical, occupational & speech/language therapies
- podiatry
- nutritional counseling and help with special dietary needs
- a variety of engaging and stimulating activities.
- Eddy DayBreak at Rensselaer -- an award-winning social day program that provides a safe, supervised environment that enables older clients to maintain an active lifestyle while still being able to enjoy the comforts of home. Winner of the 1999 New York State Intergenerational Network Program Award, this program also features creative arts therapy with on-site music, art and dance therapists to work with seniors.

- Eddy DayBreak at Marjorie Doyle Rockwell Center -- This medical day program features a caring environment designed especially for older adults experiencing memory loss. The program offers a supervised environment for adults with dementia; assistance with personal care or other activities of daily living; monitoring of medications; and care management. Nurses work closely with the participant's personal physicians to address medical concerns. Eddy DayBreak sites in Troy, Cohoes and Marjorie Doyle Rockwell Center in Cohoes accept Medicaid, while Eddy DayBreak in Rensselaer participates in several Medicaid waiver programs. Medicare and private insurances may cover some services. We would be happy to discuss fees, insurance or private pay arrangements with you.
